Person Centered Planning Is Here!

PC2 is excited to announce that we are now offering Facilitated Person Centered Planning.

What Is Person Centered Planning?

Person Centered Planning is a process of planning that is driven by the individual and is rooted in community and not a service system. Although services are very much a part of the picture, they are not the driving force or vision. As a result, person centered planning is an evolving picture of a persons strengths, skills, dreams and goals that provide a roadmap to the future in work, school, home, relationships and community life.

Person Centered Planning allows an individual to create a path to having the life that they want and the ability to have control over their own life decisions.

How It Works

Using a process called MAPPING, an individual and their team of family, friends, teachers, service providers and others, share information to build their Person Centered Plan.

What’s Included in a PCP?

  • A Path to Action Plans centered on the person’s talents, hopes and dreams
  • A facilitated process focusing on key questions and themes such as “who am I?”, “what are my goals?”, “what are my dreams?”
  • Inviting people who are a part of the individuals life. Family members, friends, teachers, service providers, neighbors, and anyone else that can help with the process.
  • Graphic recording - pictures and words for all abilities to honor the whole person.
